(above forms ~ Adobe Acrobat format) These forms are made available in English, and in Spanish. You may pick up a registration form, located in the narthex (main entrance) of the church, in the parish office during office hours, or email the parishsecretary@stlukescatholic.com and a form will be sent by mail. The forms should be completed by the parishioner with as much information as possible and returned to the parish office in person, through the mail, or give to one of the ushers at weekend masses. We'd love for you to become a member of our parish family! Other registration: children, youth and adults in the Faith Formation programs. |
